Usefulness Assessment of Smart Coverage Function in CT TAVI(Trans-catheter Aortic Valve Implantation) Examination

Abstract
Purpose: In CT TAVI Examination, it attempts to examine the usefulness of new test method that exams the whole lungs including the heart with two wide coverages by axial mode and then without losing time, that fulfills a examination sequentially from the end of lungs to femoral artery by spiral mode.

Meterial and methods: A study was fulfilled by dividing patients who went to the hospital by aortic valve disease and received CT TAVI Examination from June 2016 to July 2017 into a control group(14 patients) that the heart was examined by wide axial mode and the whole aorta was examined by spiral mode, and an experimental group(17 patients) that the whole lungs were examined by two side axial modes, and then using a part whose examination ends as a starting point, the aorta left was examined by spiral mode. To assess image quality, average HU value, SNR, and CNR of aortic arch, ascending and descending aortas, and both iliac arteries were evaluated, and additionally, general image quality was assessed with four-point criterion. Dose was assessed through CTDIvol and DLP values, non-parametric test was carried out for significance test, and 95 % confidence interval was used.

Result: In case of average HU value, aortas of the heart were all highly measured in the experimental group, and especially, this value was measured 11 % higher in aortic arch. However, both iliac arteries were highly measured in the control group. In case of SNR and CNR, aortas of the heart were all highly measured in the experimental group, and especially, SNR was 42 % and CNR was 27.8 % so they are measured the highest in aortic arch and showed a significant difference(p<0.05). In an assessment with four-point criterion, the control group was 3.5 points and the experimental group was 3.23 points so the control group was measured 0.27 point higher and both two groups are evaluated as images that are suitable to reading. As a result of measuring CTDIvol and DLP values offered by equipment to examine dose change, it was measured that CTDIvol value is 23.54¡¾8.91 in the control group and is 16.94¡¾4.07 in the experimental group, and DLP value is 610.01¡¾120.78 in the control group and is 493.61¡¾68.21 in the experimental group, 28 % of CTDIvol valueand 19 % of DLP value decreased in the experimental group,and there was a statistically significant difference(p<0.05).

Conclusion: A new test method that mixes two wide coverages with axial mode in CT TAVI Examination, expands the scope of examination up to 32 cm, exams the whole lungs including the heart and aota, and then exams abdominal aorta sequentially by spiral mode is a useful test method to reduce dose dramatically without the reduction of image quality in comparison to the existing method that exams the heart by axial mode and exams the whole aortas by spiral mode.
